After Hospitalization

We can customize a care plan to meet your needs after discharge from Surgery, Rehabilitation Facility, or Hospital.

Medicaid services are available to assist patients who need assistance with activities of daily living (ADLs), including: Eating, Dressing, Bathing, Toileting, and Mobility.
